What to Expect on Your First Visit.
On your first visit, you will receive a comprehensive evaluation including examination of teeth, soft tissues, and supporting structures. This evaluation, along with pertinent medical history and dental history, discussion of home care, and any necessary x-rays will be gathered by Dr. Jazwiecki and our team of dental hygienists.
Based on our evaluation, we will discuss with you any immediate treatments that are necessary, as well as long-term oral health plans. At this point we will also discuss options, costs, and answer any questions you may have. You can download our new patient form here.
Future Visits.
Keeping your teeth healthy and your smile bright is our primary goal, and we know that preventive care is the key to maintaining a healthy smile.
At each appointment, We will make recall appointments for you at your regular check-ups to ensure that the good oral health plan that we have developed for you is maintained in a timely manner.
Appointments & Cancellations
If appointments need to be canceled, we ask that you do so 24 hours prior to your appointment as a courtesy to our other patients.
If you have an emergency, please contact our office immediately. We will do everything we can to provide you with treatment as soon as possible to get you back to what you love most.
Payment Options.
RiverPark Family Dental participates in several dental insurance plans. Our staff is available to assist you in filing your claim. To make the process go as smoothly and quickly as possible, we recommend that you make sure that the plan you are enrolled in does not restrict which dentist you can see.
